1) One of the source of this text is devii bhaagavata. Sometimes

these types of texts may repeat in other puraaNa-s also.

The very first page has the verse "shaapa ..." explains the

consequence of non-observance of this shaapa vimochana mantra-s.

 

3) The English translation of devii bhagavata I have does not

elaborate on the details. But I was told [1] that the original text

in Sanskrit has the details.

5) I dont think all sampradayams accept this requirement of shaapa

vimochana.

 

6) Again according to some [1] this need not be said everytime, but

according others you have to (I have Sanskrit-Tamil textbook, which

says you should).

 

7) Best is to ask your family elders and do what they suggest.

 

There is nothing wrong or harm in saying a simple mental prayer for

redemption from all curses, of which you may not know anything about.

This can be in your own words.
