Seeking References of the Pancha Tattva Mantra in the Scriptures or in the writings of Shrila Thakura Bhaktivinoda and and Shrila Bhaktisiddhanta Prabhupada

I required any information available on the following queries:

 

(1) I am very desperately seeking the references of the Pancha Tattva Mantra in

the Scriptures or in the writings or purports or instructions of Shrila Thakura

Bhaktivinoda and Shrila Bhaktisiddhanta Prabhupada or any previous Acharyas. I

am aware that Shrila Prabhupada has most mercifully given this Mantra to be

chanted before the Mahamantra.

 

(2)Where has the Pancha-Tattva Mantra originated from? It is not there in Cb, Cc

or any writings of the our Acharyas as far as I know. Has it come from Shrila

Jagannatha dasa babaji Maharaja? If so what is the authentic pramana for this?

 

(3)Was it being chanted in Gaudiya Math during Shrila Bhaktisiddhanta

Prabhupada's time or by Shrila Bhaktivinoda Thakura?

 

(4) If it was not chanted what was chanted at that time?

 

(5) When was it introduced first time among the society of devotees?

 

I will be very grateful if you can answer these queries or give me a clue how to

get an answer or direct me to some appropriate devotee at the earliest.
