Typically the time of baby coming out of the womb and taking first breath are pretty close (within seconds of each other). So, the time of acquiring 'prana' was replaced by "time of birth". As for conception, we do take the timing of coitus, when the seed falls in the womb, and calculate the good and bad of the foetus in the womb according to that nisheka lagna.
