You can read about it in Satyaraja´s book on the life of Lord Caitanya. His disappearance was in 1534 (I don´t recall if the date is mentioned anywhere). And the place is in Orissa. And two different versions exist. One is that He merged into the form of Lord Jagannatha and the other is that He merged into the form of Tota Gopinatha. Your servant, Ramesvar Das.
