I´ve learned otherwise, madhyam adhikaris can also initiate. If only uttama adhikaris can, ISKCON would be very high-ranking. Uttama is something very, very rare. Bhagavad-gita 4,34 says: jnaninah tattva-darsinah: selfrealized seers of the truth. Prabhupada calls such person a spiritual master in his translation. Anyway, uttama means transcendental, so there may be some twist as to when a person is transcendental (to tamas) and thereby uttama. Another point is that the initiation is a procedure where we link with Krishna VIA the medium of guru. Guru is the external manifestation of Krishna within.
