If I remember properly it is from the notes on his life by HH Bhakti Vikasa Maharaja (from Maharaja's forthcoming book). Or it is from the www.veda.harekrsna.cz web-site hosted by bh.Jan Mares (NE - BBT Czech). I'm not sure as I've been downlading both and then editing them in WinWord in order to print them for myself.
