The difference is that the uttama adhikari who is acting as a madhyama adhikari is meditating on the pastimes and service of the Lord within himself and is often directly engaged in that service within his mind, whereas a factual madhyama adhikari is on the stage where he is just coming to realize this service and hoping to attain such an exalted position. The uttama bhakta will not reveal this to others because that is not done usually, and will always present himself as meak and humble as if he were a factual madhyama adhikari. The preaching potency of an uttama adhikary is far superior to a madhyama adhikari, and this is obvious.
