Here is one interesting analysis for your inspiration.

 

Acording to the Padma Purana and pancaratrika-viddhi when one has been purified by 1) performing of austerity, 2) wearing Visnu tilaka and has 3) received the holy name of Krsna is considered to be a kanistha adhikari.

The madhyam adhikari is one who has performed two aditional purificatory processes 4) receiving the gayatri mantra initiation and is 5) performing yajna for the sake of Lord Visnu or Krsna.

A spiritually developed person who has mastered the above-mentioned five processes of purification (panca-samskaras), who is absorbed in the nine devotional activities (deity worship, mantra, yoga, yajna, prayers, nama-sankirtana, service, and worship of both the Vaisnavas and the Lord) and who understands the esoteric meaning of these different activities in full, is to be known as a maha-bhagavata, a great devotee of Visnu, and a topmost Vaisnava. (Padma Purana)

This classification from Padma Purana is quoted in Hari-bhakti-vilasa and doesn't seem to correspond with the two other classifications that we know. A search for "madhyama" in the files of the text at grantha mandira site should make the passage easily recoverable. And the Padma purana chapter and verse reference numbers may be there.
