The following mantra is not complete as the mantra for Vishudhi and Agya chakra is missing.

 

Sarir me sthapan Establish Guru onto yur body

Hridyaya mein dahine haath

 

Shri Shiva Nanda Nath Para Satyamba Muladhar Chakre Sthapayami Namah

Shri Sada shiva Nanda nath chtit satyamba Swadisthan Chakre Sthapayami Namah

Shri Iswarananda Nath Ananda Satyamba Manipur Chakre Sthapayami Namah

Shri Rudra deva Nanda Nath Ichhya Satyamba Anahat Chakre Sthapayami Namah

???

Shri Vishnu Deva Nanda natha Kriya styamba Sahsrar Chakre Sthapayami Namah

 

 

I enquired from Shasri ji and gurudham also but could get only mantra of Vishudhi chakra can any body provide mantra for Agya Chakra.
