It sounds like panic disorder to me, or a variation on it... the thing that cues me off to it is that they wake you up in the middle of the night (which seems to indicate a brain chemistry imbalance thing). There's a real chance that a mild dosage of something like Prozac or another one of the SSRI antidepressants (Zoloft, Paxil, etc) might help. Have you ever considered it? Even 6 months on a relatively low dose can " work miracles " -- it did for me back in 1988 when i had something similar going on.
