Surya and Upapada Surya being in twelfth house is the clear prediction for the problems with the sigth and being lord of Upapada fulfills the first condition. Shukra and Rahu Both Shukra and Rahu aspects Simha-Upapada by Rasi drsti whilst Shukra also aspects twelfth house and its lord.  Person is medically certified blind person. Blindness for left eye, and 40 percent blindness for right eye.
