need charts of pharmacists, MD that prescribe chemotherapy, people with serious thyroid disease

Dear friends,

If you have any charts of persons who are

1) practicing pharmacists in USA. please send me the chart detials.Just include

the persons inials, do not send me the chart with their name on it.PLease only

send data that is accurate, no guessing please.

OR

2) Doctors who utilize/prescribe chemo therapy for cancer patients.

3) Charts of women and men with serious thyroid disease and preferably not any

other afflictions.

If possible give the date the disease started to create problems.
