The 'points' David mentions are very intuitive. They are level with the eyes and about an inch around the sides of the head from the eyes, actually this is a very sensitive area, and just by pressing lightly in that area you will definately find the 'hot spot'. As for your second question, you basically grind the teeth from side to side. It is like when you have a new filling and the Dentist ask's you to bite up and down and SIDE TO SIDE. It is this side to side motion of the jaw and teeth, you dont need to do it hard, just in a natural way. While doing this, as you say you keep the mouth closed, and looking into the 'third eye' with eyes closed, silently chant SA TA NA MA. So grind to one side SA, then to the other TA, then back with NA, then to the other side with MA and so on.
