It was published in Varanasi in 1992 by Sampuranada Visvavidyalayasya Yogaratnagranthamala (Madhu Khanna references a published version in one of her essays), but it's not even listed in WorldCat. I suspect it was a very limited publication. Does anyone have a copy they can scan or copy for me? Try the database of UT Austin.
