You control the pain with morphine until the final day of the experiment, when you replace the morphine with salinesolution. Guess what? The saline takes the pain away. Hm - I have a patent currently whose pain is not controlled by 100mg Oxycontin BD. Pain is worse in the evening. So GP in order to try & assist this patient more accurately with the greater p.m. need, changed the doses to 80mg in the morning and 160mg earlier in the p.m. - increasing the overall dose by 40mg. Today i find the patient hysterical first thing in the morning. She sobs "Dr has reduced my morning dose!" I explain in vain that she is receiving more overall. We decide to order 2 x 40mg tabs for the morning dose, hoping that the psychological need for MORE will be appeased by receiving 2 tabs in the a.m. instead of one. " When the monkey trainer was handing out acorns he said " You get three in the morning and four at night " This made all the monkeys furious . " Well then " he said " you get four in the morning and three at night " The monkeys were all delighted. There was no change in the reality behind the words , yet the monkeys responded with joy and anger. Let them if they want to .
