Magnesium, Peripheral Neuropathy and HIV

An estimated one million people are currently living with HIV in the United States, with approximately 40,000 new infections occurring each year.

As part of a larger study on self-care for symptoms in HIV disease, this study

analyzed the prevalence and characteristics of peripheral neuropathy in

HIV disease, sociodemographic and disease-related correlates and

self-care strategies. Peripheral neuropathy is the most common

neurological complication of HIV and is often associated with

antiretroviral therapy (pharmacological drug therapy).[ii] This supports and expands our research on magnesium and peripheral neuropathy. It may also provide yet another more effective treatment option of interest to a specific disease group.

Results

of the study indicated that respondents with peripheral neuropathy

identified 20 self-care behaviors including complementary therapies,

use of medications, exercise and rest and/or elevation of extremities.

An activities checklist summarized into five categories of self-care

behaviors including activities/thoughts, exercise, medications,

complementary therapies and substance use was used to determine

self-care behaviors. Taking a hot bath was the most frequent strategy

used by those with peripheral neuropathy and received the highest

overall rating of effectiveness of any self-management strategies

included in this study at 8.1 (scale 1-10). Also included on this “top

20” list is the use of magnesium, massage and even marijuana. Sounds like a bath with magnesium oil would reach a place high on this list. It

may not cure HIV, but would be a viable and effective palliative

therapy for these devastating effects caused by the antiretrovirals,

that appear to be causing substantial harm themselves.

They

have produced in humans a new category of acquired mitochondrial toxins

that cause clinical manifestations resembling the genetic mitochondrial

disorders

Mitochondria

are the only organelle other than cell nucleus with its own DNA.

Mitochondria lack an adequate DNA-repair mechanism so this is

devastating. Hereditary Mitochondrial disorders include

Cardiomyopathy, optic nerve neuropathies, deafness, diabetes,

respiratory conditions, and exercise intolerance disorders. We are also

seeing how heavy metals are affecting the DNA and Chromosomes, either

by causing mutations or by causing their inherent self-repair systems

to become dysfunctional.

I

would also think that the use of anti-epileptic drugs, which were also

included on this top 20 list, would be able to be reduced. Some

anti-epileptic drugs, such as Neurontin, are frequently used to treat

the pain of peripheral neuropathy, but add their own toxic burden to

the suffering body.

And in fact the recommendation was made as far back as 1999 that:

A

simple regimen of calcium, magnesium, and vitamin B6 appears to prevent

and treat peripheral neuropathy, a side effect of some drugs used to

combat AIDS. Although the treatment has not been proven effective,

anecdotal reports indicate that it works, and has few risks. Doctors

can also use certain prescription drugs to alleviate the discomfort

associated with neuropathy. In cases of severe neuropathy, doses of

drugs may be reduced or stopped to prevent lasting nerve damage.[iii]

The

use of IV magnesium sulfate has been shown to decrease the need for

pain medications (morphine) in cancer patients. And some oral magnesium

agents have been shown to reverse peripheral neuropathy.

The

benefits of magnesium treatment have been proven by Dr. Herbert

Mannsman, Director of the Magnesium Research Lab, who has been able to

reverse the effects and pain of peripheral neuropathy related to type 2

diabetes, which he suffers from. He utilized high doses

and up to 24 tablets/day of oral preparations of magnesium to turn his

debilitating problem around. Both autonomic and peripheral neuropathies

are associated with Diabetes.

Peripheral

Neuropathy is only one of 100 different forms of neuropathy that are

associated with blood vessel and nerve tissue damage from many causes. Peripheral

neuropathy (PN), associated with diabetes, neurotoxic chemotherapy,

human immunodeficiency virus (HIV)/antiretroviral drugs, statin drugs,

alcoholism, nutrient deficiencies, heavy metal toxicity, and other

etiologies, results in significant morbidity.

It is frequently found associated with injury, medication effects,

autoimmune disorders, chemically caused disorders, infections and can

also involve more than just the peripheral nerves, in a condition

called polyneuropathy. An excellent overview of the

neuropathies can be found at The Peripheral Neuropathy Fact Sheet, from

the National Institute of Neurological Disorders and Stroke where you

will find types, causes and symptoms many of which may surprise you. The

neuropathies are often an early sign that a disease process is taking

hold in our bodies and the reason people first seek out medical

evaluation:

Long

term Mg supplementation is able to restore a normal Mg status and

influence favorably the natural evolution of polyneuropathy as compared

to non supplemented Type 1 diabetes controls, is the conclusion of a

study done by The Department of Diabetology, Metabolism and Clinical nutrition, University Hospital Antwerp, Belgium in 2004.[iv]

The

IMVA has focused special attention on the benefits of magnesium to the

blood vessels and nerves related to peripheral neuropathy and diabetes,

which today leads to one limb amputation every minute.[v],[vi] However,

the information presented applies to most forms of neuropathies, both

in the alleviating of symptoms of pain, numbness and total lack of

sensation, to addressing the underlying causes themselves, which

frequently are directly associated with magnesium deficiencies and can

be improved by maintaining adequate magnesium levels.

The

medical community is more concerned with pharmaceutically manufactured

drugs in their treatments of the families of neuropathies, and fail to

recognize or recommend magnesium as an essential and effective

treatment, which should precede the use of more toxic drugs and

may prevent the drastic suffering and surgeries that often result.

While there is controversy over the causes of HIV, the fact remains

that neuropathy occurs and this should be addressed in the most humane

way possible.

Healthy nerves and blood vessels require magnesium.

We can relieve much pain and suffering if we raise magnesium awareness to the level it belongs……to the top of the list.

Symptom management and self-care for peripheral neuropathy in HIV/AIDS. AIDS Care. 2007 Feb;19(2):179-89

[ii] Patients

treated with nucleoside analogue reverse transcriptase inhibitors

(NRTIs) develop a varying degree of myopathy or neuropathy after

long-term therapy. Zidovudine (AZT) causes myopathy; zalcitabine (ddC),

didanosine (ddI) and lamuvidine (3TC) cause neuropathy; stavudine (d4T)

and fialuridine (FIAU) cause neuropathy or myopathy and lactic

acidosis. In vitro, NRTIs inhibit the gamma-DNA polymerase,

responsible for replication of mtDNA (mitochondrial DNA), and cause

mtDNA dysfunction. In vivo, patients treated with AZT, the best

studied NRTI, develop a mitochondrial myopathy with mtDNA depletion,

deficiency of COX (complex IV), intracellular fat accumulation,

high lactate production and marked phosphocreatine depletion. The

tissue distribution of phosphorylases responsible for phosphorylation

of NRTIs relates to their selective tissue toxicity. The myopathy is

characterized by muscle wasting, myalgia, fatigue, weakness and

elevation of CK. The neuropathy is painful, sensory and axonal.

NRTI-induced mitochondrial dysfunction has an influence on the clinical

application of these agents, especially at high doses and when

combined. They have produced in humans a new category of acquired

mitochondrial toxins that cause clinical manifestations resembling the

genetic mitochondrial disorders. Peripheral neuropathy and antiretroviral drugs; Marinos C. Dalakas; Peripher Nerv Syst. 2001 Mar;6(1):14-20 Entrez Pubmed
