1. Ayurveda offers herbal ghrutams to be used for local application and

also basti to strengthen pelvic floor muscles for elastic adjustments

during birthing process. This is to ensure that no scalpel or forceps

is needed during birth process. Elaborate post-natal protocol streching

to 42 days, involving food, lifestyle, massage and herbal formulations

to return the repro system back to normal is also in practice, in

villages where tradition is not yet bull-dozed by modern medicine and

practitioners, e.g. see message#14533

 

2. The problems described by you come predominantly under Vata

category, wherein body's in-built healing program is disturbed. The

pelvic cavity contains most complex network of nerves, which gets

disturbed by childbirth mechanical intervention. Ayurveda offers

ghrutams for local application and also bastis to hasten the healing,

regeneration of cells, and restore nutrient circulation channels

(strotas) so that pain, swellings is reduced and elasticity is

restored. The choice of herbs or mixtures of herbs and ghrutams need to

be patient-specific, as mom is under rapid transit of dosha spectrum.

Specific food choices to reduce the pain, inflamation, bleeding and

hasten the healing are to be made. Though normal protocol streches to

42 days, the healing essentially completes in 4 weeks.

 

3. Problem described by you can be cured provided patient is willing to

take patient-specific formulations and limited panchkarma procedure,

which is again patient-specific, under supervision.

In early August 2008, the patient gave birth by forceps resulting in

a third degree perineal tear. The tear was sutured at the time

but suturing was unsuccessful, resulting in failed union of the

pelvic floor muscles at the perineum and the external anal sphincter.

 

An ultrasound examination of the rectum found much scar tissue at the

site of the anal sphincter tear, and this is thought to be the cause of

pain and occasional bleeding from the rectum on passing stool. There

is increased urgency to go to the toilet, but no urinary or

fecal incontinence.

I fully agree with Dr. Bhate .

The persons's own & unique constitution is important to treat the

cause . A certain period is required to treat the problem under

supervision .

in the present situation i think combination of haridra, aloe and trifla should

work as haridra heal the wounds and is known anti biotic and anti septic besides

improving immunity. aloe will ensure proper cleaning and enhance the anti biotic

effect and trifla will act as astringent there by improving rectocele and at

same time will loosen the stool which will reduce the pressure on the pelvic

floor. progress can be watched.

Dear Dr Bhate !

Thanks again for your fine advice. The lady, as mentioned, has quite a

deep connection with Ayurveda and would be interested to know any referrals

or comment as to whether the treatment of this kind of condition might

fall with in the competency of general vaidya skills or whether some

special skills might be needed.
