Box 27940 Los Angeles, CA 90027 The book is $15.95 plus $2.95 shipping. If you live in California, there is also $1.56 sales tax. This information comes from a bookmark included with my copy of the book. These folks are a small printer who print as the book is ordered. Amazon had used copies starting for around $60. Order directly from the publisher if you want to save money. I found a PDF file with an order form on the web. Since I was not sure if they were still in business, I ordered from Barnes and Noble. The book did arrive OK this evening and I had placed my order on Monday. The publisher IS still in business so an order will most likely go through fine.
