I think actually, it's something which will change with time and experience as we work with clients and decide in which order it feels natural to work through the information and the best way to discuss, for example, contra-indications with clients. I have completed my basic forms however and am pleased with how they are. If you would like, I can email you over a copy and you can have a look at them for style and content or if you want to bounce any queries or feedback off someone, I'm just at the end of the keyboard!
