Medical Background: Retractile testis is a testicle that's located high in the scrotum or up near the groin. Unlike an undescended testicle, which is fixed in place, a retractile testicle can move down into the scrotum. Retractile testis is common in pre-adolescent boys. A retractile testis is one that is pulled up into the neck of the scrotum by an excessively active cremasteric reflex. It appears to be incompletely descended but may be noted in its correct position after a warm bath, or after squatting. The cremasteric reflex is dependent upon the nerve roots L1 and L2.This reflex is elicited by stroking the superior and medial part of the thigh in a downward direction. The normal response in males is a contraction of the cremasteric muscle that pulls up the scrotum and testis on the side stroked.In children, the reflex may be overexaggerated and this can lead to the mistaken diagnosis of undescended testes. Upper and lower motor neurone disorders can cause an absence of the cremasteric reflex. Temperature fluctuations in the testicles normally cause changes in the scrotum. For example, in cold temperatures, the scrotum tends to shrink in size to keep the testicles warmer. In warm weather or during exercise, the scrotum becomes more soft and pendulous, allowing the testicles to remain cooler than the core body temperature. In many boys, this shrinking effect of the scrotum tends to pull the testicle high up in the scrotum most of the time. This can occur on one or both sides. But the testicle also moves down into the bottom of the scrotum when warm. In a true undescended testicle, the testicle doesn't move down into the scrotum, even when warm. Undescended testicles typically require surgery, especially in boys older than age 1 or 2. But treatment is rarely needed for a retractile testis. After puberty, a retractile testicle makes sperm and testosterone normally. Some young boys may be embarrassed because their scrotum appears smaller than that of an older or more mature boy. The doctor have advised that the boy is having Retractile Testis. He does not have Undescended - Cryptorchidism which is more critical and has advised the parents to wait till Feb 2005.
