Given below is a brief summary of his ailment, his present condition and my questions. Brief Summary of the case Rishi Kumar Mishra is 76 year old hypertensive, nondiabetic, ex-ethanolic, tobacco chewer gentleman who had h/o recurrent TIA with MCA stenosis and h/o Ca prostate treated with local RT followed by hormonal therapy for one year with Leuprolide in 2002-03. He had rapidly developing anemia with melaena in November 2006 for which UGI endoscopy was done and showed mass in stomach. Biopsy from gastric mass suggested moderately differentiated adenocarcinoma. He underwent radical total gastrectomy with emblock upper abdominal lymphnode dissection with roux-en-y esophagojejunostomy in 14th December 2006 in MSKCC, USA. HPE (19.12.06) suggested invasive moderately differentiated adenocarcinoma of middle 1/3rd of lesser curvature of stomach invading up to serosa, vascular and perineural invasion present, 13/32 lymph node was positive for malignant cells, all margins free. Post operative CT abdomen (16.02.07) showed no e/o residual disease. He received EBRT to a dose of 45Gy/25frs from 20.03.07 to 21.04.07 along with 6 cycles of 5-FU-based concurrent chemotherapy. Subsequently, following MRI abdomen and PET CT in July 2007 which showed metastatic hepatic mass in segment, V, VII, he received Xeloda, Oxaliplatin and Epirubicin based chemotherapy till December 2007. MRI abdomen in February 2008 showed increase in the size of residual hepatic mass with left basal pleural effusion s/o progression. Then he was planned for Taxotere and Irinocam based chemotherapy which he had received till April 2008. MRI abdomen (April 08) showed heptic lesions minimally increased in size with bilateral pleural effusion. Then he had taken alternative treatment till June 2008. USG abdomen (25.07.08) showed metastatic lesion involving whole of liver with ascites, bilateral pleural and pericardial effusion ssssss s/o progressive disease. He was then planned for 4 doses of weekly Taxol, 1st of which he received in Jaslok hospital, Mumbai on 30.07.08. He came to RGCI for 2nd dose of weekly Taxol, which he received on 08.08.08. Present status 1. Apart from chemotherapy, he has been taking Globac-Z and Calcimax. 2. After he started taking Sutherlandia OPC capsules (two per day) on August 9, 2008, he feels dryness in the throat. 3. After the last two rounds of chemotherapy, he is having either constipation or loose motions. 4. He is taking pomegranate and carrot juice, and wheat grass daily. 5. His Albumin and Protein are very low ever since the surgery. At present the doctors have advised him to take whey protein (freshly made from milk). He takes the liquid part of it, not the cheese part. 6. His hemoglobin and WBC counts are low. After chemotherapy, the doctors have given him two injections of growth factor (Grenocides) 7. Mr. Mishra is a strict vegetarian. 8. He eats blend food at regular intervals and is able to work for about 2-3 hours every day (meaning he is not bedridden). Questions 1. Can you please suggest any herbal laxative? 2. Can you suggest any dietary modifications? 3. Because of his stomach conditions, sometimes capsules get passed out without even opening, according to the doctors. Do you suggest it will be fine if we change from Sutherlandia OPC capsule to Sutherlandia OPC liquid once we finish the bottles we have already ordered? I will leave your questions for Marc, but I will observe that your husband must be a tremendous fighter who has done a lot of good things to have survived all of the horrible effects of the many rounds of chemo and the surgery he has had.
