Or if anyone has created a template that they would be willing to share? I know there is electronic billing software available - but it is very expensive. This is a fillable form but you will need acrobat from adobe to use it. I am waiting for them to post the new CMS form soon as things are changing. I hope this works for you. Mike W. Make one original with everything filled in except for the Date of Service, the date you sign at the bottom of the form, and the prices. Keep that original in the chart and make 3 or 4 copies of it. On the day of treatment, fill in the few numbers that aren't there and send it in. It takes less than 15 seconds. Some companies now, though, are requiring " typewritten " HICFs so I have a billing person for that. I suppose eventually I'll have to join the electronic fold and do it all that way. Guest guest Posted April 24, 2007 Report Share Posted April 24, 2007 Just a few of things to be aware of... 1. The HCFA 1500 form has been replaced by the CMS 1500 form. While carriers are likely to continue to accept the old forms for non- Medicare claims in the near term, there may be a point when claims on the old form are rejected by certain carriers. 2. If you are buying claims printing software, be sure to buy software that supports both the old HCFA and new CMS formats. A simple Google search on " CMS 1500 software " will turn up loads of appropriate claim filler software (usually in the $50 range). 3. Any electronically transmitted (non-paper) claims should now use the new HIPAA-compliant CMS 1500 format. 4. To be fully CMS-compliant, the forms must be printed with special inks; printed PDF docs are not. This may not be an issue for non- Medicare claims, but it's something to be aware of as each carrier will establish their own claims formatting and submission requirements. --Bill.
