Canker sores are different from cold sores. Cold sores (a.k.a. fever blisters) develop on the outside of the mouth and face, while canker sores occur inside the oral cavity. The peak occurrence for canker sores is during adolescence and young adulthood. The older we get, the less frequent they are. Why? Canker sores tend to be directly related to nutrition and unlike cold sores they are not contagious. HOW DO WE GET THESE SORES? Canker sores seem to be a direct result of our immune system identifying a foreign chemical molecule and the release of lymphocytes (white blood cells). When the lymphocytes attack the foreign molecules an ulcer is formed. You’ve got an army of white blood cells attacking a foreign substance and the battlefield is essentially the mouth ulcer. Cold sores are completely different and are caused by a virus called herpes simplex type 1. Somewhere, usually during childhood, you came in contact with a person carrying this virus. If you shared eating utensils or towels or were kissed by a person with this virus, you usually contracted it. This virus can lay dormant for years waiting for the opportune moment to be reactivated. When it rears its ugly head it appears as a sore on the lips, mouth, nose, chin or cheeks. WHAT CAUSES A CANKER SORE OUTBREAK? With canker sores, research suggests the lack of nutrition and some chemical agents to promote the sores. Poor diets devoid of; vitamins B & C as well as zinc, folic acid, iron, selenium and calcium contribute to the frequency of canker sores. Stress seems to play a role as well, further depleting our bodies of nutrition. Females may experience more frequent outbreaks associated with their menstrual cycle and find a remission from outbreaks during pregnancy. Other research points the finger at chemicals such as foaming agents found in mouthwash and toothpaste known as SLS (sodium lauryl sulfate). These chemicals seem to dry out the oral tissues and deteriorate the protective layer and compromise the more vulnerable tissue underneath, which is sensitive to acidic foods. The research further reported that participants that used SLS-free toothpaste experienced fewer canker sore outbreaks by 81%. Those who did have canker sores reported the sores were less painful and healed faster. Other chemicals that seem to promote canker sores are food additives such as flavoring agents (cinnamonaldehyde) and food preservatives (benzoic acid). Some drugs also encourage canker sores such as nonsteroidal anti-inflammatory medicines (NSAID’s), beta blockers and chemo-therapy agents. Also, people with food allergies to wheat, dairy, fish and nuts seem to experience more canker sores. Some prescribed medications can also precede a canker sore outbreak. Physical trauma can also trigger canker sores. The trauma can be biting your lip accidentally or the irritation of orthodontic braces to cause a canker sore. Studies suggest that nearly 40% of canker sores are from oral trauma. Genetically you may be more likely to have canker sores if your parents did (35%) or if you are an identical (91%) or fraternal (57%) twin. 38 Finally, an underlying systemic disease could be the cause of frequent canker sore outbreaks. WHAT CAUSES COLD SORE OUTBREAKS? Usually emotional or physical stress gives the cold sore virus the opportunity to rear its ugly head. It likes to sit dormant on the nervous system, which acts like an alarm clock to the virus when we get stressed. Medicine has no known cure but can only try to minimize the occurrence of outbreaks. MEDICAL TREATMENTS Modern medicine recommends steroid salves or Tagamet to try and decrease acid production in the stomach; however the FDA has not approved the use of heartburn medications (cimetidine) for canker sore treatment. Cold sores usually heal within 7-20 days and medications do not cure the virus but simply try to reduce the duration and pain of the outbreak. Doctors will prescribe antiviral agents, anti-inflammatories, antibiotics and anesthetic ointments.
