superbill for workers comp?

Hi all,

 

I have offered superbills to patients who have health insurance which covers

acupuncture, so they can receive reimbursement for fees they pay me.  Is there a

corollary form available for workers comp patients?  If so, what is it called,

and how can I get a template?  I am in Arizona.
