Tax ID# on subforms: follow-up

I have contacted the NCDOI (NC Dept of Insur) regarding the issue of the

insur co (blue cross and united health care) requiring my tax id # on a

subform when the insur co is cutting the check to the patient and is not

paying me. The rep i spoke to said in NC there is no legally statute saying

anything on this issue, so it is not a legal requirement (contrary to what

sam collins of acu today advised) that the practitioner provide a tax id# on

the subform.

 

he advised me to fill out a complaint form on their website and send it to

the NCDOI, who will pursue the matter with the insur co as to what their

internal reason is for requiring it. blue cross has contacted me in writing

asking for the tax id #, so i'm am filing a complaint against them.
