I usually consider what kind of company they are; if they are a top payer and I am very motivated to retain the client, I just comply, but if they are on the low end of the spectrum, I usually just copy the progress notes and send them in. If my notes are aspects of a legal proceeding, e.g.. it's a car accident, I make an extra effort to type everything nicely and keep very extensive notes, etc, etc, and yes, it takes additional time to collect all this information and I do bill for it. When the companies ask for the copy of the file, I charge $45 for the copy, which is a formally regulated fee in my state. The last time I went to see my MD she came in with her laptop and did all notes in my presence, and I am sure she chooses to do it that way is because she can bill for all time she has direct face contact with me, but not for anything else. Guest guest Posted February 27, 2010 Report Share Posted February 27, 2010 so, IS it legal to charge for our notes? how do we do it, exactly? i am not in network with insurers, but my patients do submit out of network claims, and i sometimes get requests for my notes....and it does take a while because i re-copy them without any of the extra information that is not pertinent to the chief complaint (at least in their eyes). In Oregon Workers Comp sets up fees for all services every year, and those fees are generally seen as " customary and Usual " The copy of the whole file has a price tag of about $45, and I routinely charge that for all attorneys and Workers Comp. Insurance companies can and sometimes want an progress note for a specific date, and will often tell you that they will not process the claim unless you provide it, and I just send it in without charging. (if you do that make sure it's disclosed in your HIPPA form. It's more tricky when clients or other providers ask. There is a chiro here in town who asks for a complete file every time one of my clients went to seem him , and I got tired of it and told him he needs to send $ 45 with the request, and he is furious. If clients move out of state and have been with me for a long time and want their file, I usually just gave it to them. Charging another provider for consultation on a patient seems wrong. You can usually submit a summary of your case and that should work, it also seems a bit off that this provider asks for all the patient notes on every acupuncture patient. You should not have to pay for copy each and every time either. Suggest you send the patient to a different chiro next time. I hope you keep a copy of your out of state patient files, just to CYA.
