Onset July 2009. To date, the patient has had 3 UC " flares " 2 of which were handled in the hospital (prednisone, prevacid, pentasa, IV steroids…) with this last one being solely handled with chinese herbs and diet. Flares: Have all been preceded by viral pathogen. Constipation has also preceded flares. One flare also had introduced bifidus just before flare. Patient has seen Naturopaths, Dieticians, DOM's, GI's…The next step from the GI specialist is to go on sulfasalazine, immuron and 6mg prednisone every other day for up to 1 year. If that didn't work they will recommend surgery. History: Vaginal birth IV antibiotics for 3 days when first born Full vaccinations Severe cradle cap as child Reflux as baby, colic, burping curdled formula (Enfamil/SMA) (breastfed 4 months) Started solids at 6 months Always picky eater Diaper rash which was treated with hydrocortisone Eczema. No asthma. Allergic to cats (itchy on abdomen). Diagnosed with stomach erosion Anemic Recent stool analysis positive for unknown taxonomy parasite and low levels of pancreatic enzymes Last year had odd limping which doctor said was from virus and would go away and did. Presentation: Dark circles under eyes Pale complexion Cold hands/nose Night sweats (worse with prednisone, but there presently as well without medications) Low appetite Parents report: Some what fearful. More so now with doctors and treatments. Has become quieter. 3 small bumps in a line on index finger (I have thought they were flat warts/viral) GI: 1-2 bm's/day. Will occasionally skip a day. Not much abdominal pain. BM's vary between mushy and fully formed. Have become more formed with Chinese herbs and diet. Parents report stomach bloating. Increased burps, bloating and hiccups recently. Some blood streaked bm's for past month. Some mucus with stool. Stool often comes out formed at first and then mushy at the end. Patient came to my office Jan. 2010. I have only seen her in person 1 time. She lives in Canada. She was not in a flare when she came in. She was on prednisone, prevacid and pentasa. She finished her taper of prednisone on 2/17/2010. On 2/19/2010 she had blood streaked bm. Parents also reported that she had slight cold at this time and had been sniffling on and off for 2 weeks. (They reported that during the last flare she had bled a small amount for 1 week and then blood started pouring out). She has basically had a low lying cold for past month. The bleeding has just been drops and will go days without any blood. When her bm is harder, the bleeding will come back. When her stool is mushy, there is usually no bleeding. Herbal Treatments: Acute bleeding: Yunnan paiyao Kan: Hemorrease (Huai Jiao Wan) stop bleeding, disperse wind, clear heat and damp Far East Summit: Hemostatic Coptis root 7 Forests: Imperata (stop bleeding, clear heat) 7 Forests: Picrorrhiza (similar Bai Tou Weng formula) External Wind-cold: Gentle Warriors: Windbreaker, Chest Relief, Pipe Cleaner Chronic UC tx: 7 Forests Atractylodes (based on Jianpi Ling) Gentle Warriors: Grow and Thrive Pain: 7 Forests Peony formula as needed for pain (really helps) She is currently taking yunnan baiyao, hemorrease, atractylodes, imperata, and picrorrhiza. She continues to have small amounts of blood but not a full flare with diarrhea and severe bleeding. I was treating her with the idea that I needed to nurture spleen qi and not be too draining. Since going to Alex Tiberi's pediatric seminar last month, I started her on the picrorrhiza formula with the idea of clearing more heat. I would like to get her off of the yunnan paiyao as I'm not sure how long it is safe to keep her on it or if it is even necessary. I have again recommended that they see someone regularly for treatments and do herbal enemas. Update 3/26/2010: When we decrease the yunnan paiyao the bleeding increases. She is now having increased amounts of blood with every stool (still 1 stool/day) and today there was a large clot. Not much mucus. They are now willing to try an herbal enema tonight. It seems to me that it is a necessary, required aspect of her treatment. I agree with the idea of boosting the Sp Qi...depending on her presentation, Gui Pi, Bu Zhong Yi Qi, Bu Pi Yi Chang or some similar formula might act as a base rather than Bai Tou Weng...What is your reasoning for the Bai Tou Weng (and other heat-clearing formulas)? I do not see, in your report, any heat signs. Everythign you have reported is deficient cold and damp with qi stagnation. What colour is her stool? What colour is the mucous? What type of smell does her stool have? As a minimum does the stool have a foul smell? Does she have irritation aorund her anus? Does she have a visible blood vessel on the palmar aspect of either index finger? If so, how far does it extend and what colour is it? In any case, without heat signs I wouldn't be able to justify the formulas you've chosen...but I am probably missing something. But be careful, I agree that you'd want to be very careful of depleting this child further. Thanks! I'm not familiar with the 7 forest formulas and you do have the coptis in there...but seems like there is a critter still trapped in there that needs to come out....what was the cause of onset? I assume you have also looked at food sensitivities and the diet in general. Her stool analysis came back with a positive parasite. I'm concerned about the treatment for this and her spleen qi. I believe the cause of onset is viral. Yes, she is on a strict diet and doing well with it. Again, I appreciate all of your help with this. Other: Vitamin D, floradix liquid iron, multivitamin, chlorophyll, fish oil She has taken l-glutamine, psyllium seed, zinc, omega 3's in the past She is also going to start with some metagenics inflammex and some caprylic acid. Antibiotic and steroid free chicken, meat, fish. Warming soups. The whole deal.
