If I can just think out loud about this case - it is very interesting:

 

Body hair is lung related and head hair is kidney related.

Is the tinnitus ringing (liver related) or rushing water (kidney related)?

She may not have any complaints about her period but the details about it are

important. She may have some defensiveness about her menstrual function

since she forgot about that tampon.

 

Can she be persuaded to stop smoking?

 

Do you think the hair can be restored or are you working to ease the stress

resulting from the hairloss?

Sean, I too wondered about that tampon. You said it might have been in two

months. Did she have a period during those two months? Wouldn't she have

noticed it? These are not burning issues relative to your case, but I'm just

curious. I agree with last respondent, details of the menstrual period are

very important to the diagnosis: blood deficiency? blood stagnation? You have

a lot of blood movers in the formula. How long has she been coming to you?

What brought her to you at this time, since her problem has been existing for

almost three years. Interesting case. My dermatology specialist at Yo San

says that alopecia universalis is very difficult to treat with Chinese

medicine, but maybe someone else in the group has good experience.

A 39 y/o female presented with total hairloss, no head or body hair. Three years ago this coming August she went through a period of time (about a month and a half) where she had recurrent strep throat infections. She was put on antibiotics to treat the strep. After the strep infections she had a severe flu-like illness. A couple of weeks into the illness, a tampon dislodged from her vagina. She was unsure how long it had been there, could have been two months. Her doctors attributed her illness to Toxic Shock Syndrome. After the tampon came out she felt much better, but not long afterwards she lost all the hair on her body with a western Dx of Alopecia Universalis. Western docs think it is an Autoimmune disease. Here is my intake:

She often has a low appetite and nausea which is exacerbated by often feeling stressed and anxious.

Low thirst: has to force herself to drink most of the time

Digestion: Fluctuates with nervousness/stress, easily disrupted

Bowel Movements: tends towards diarrhea with stress

Ears: tinnitus ever since illness, although it is decreasing now

Sleep: bouts of insomnia, sometimes problems staying asleep or falling asleep

Periods: no problems to report

She has had two children, divorced a year ago, is dating now but is embarrased by her hairloss.

She smokes, drinks two cups of coffee/day, and 2-3 glasses/wine/day, never cooks

Meds: just started Paxil 20mg-having palpitations as s/e, she has tried other antidepressants but has had allergic reactions to them

Pulse: R cun tight, R guan soft, chi small especially on right, Left guan tight, slight rapid

Tongue: Red edges , no fur on edge, red center with less fur

Dx: Liver invading Sp/St, Liv yin Def

Kidney Jing Deficiency?

 

I was having problems coming up with a formula, I was afraid cloying herbs for the Kidney would upset her stomach, but I decided to go ahead and try. If it doesn't I will back pedal and just work on stabilizing her Liv/Sp/St. I work with Lotus powders and put them in to capsules for her. The amounts are in grams.

Chi Shao 5

Chuan Xiong 5

Tao Ren 5

Da Zao 5

Bai Shao 5

He Shou Wu 5

niu xi 4

xiao yao san 10

qi ju di huang Wan10

 

Any help would be appreciated :)

I did ask about her periods ...they were every 26-27 days, flow 3-7 days, no clotting, cramping, PMS- one month sadness, next irritable. She charted this and found that she regularly fluctuates between the two emotions month to month. She didn't have any problems talking about her menstrual function, as far as I could tell. I don't know if she had a period during those two months. I will ask on her next visit. I have only seen her once, and she came to me after doing some research which led her to believe Chinese Med could help. She said the general stats are that it resolves for 80% of the people that get it, but the longer you have it the less likely it will resolve. A woman in her support group had her hair grow back after 7 years.

I told her that her nervousness/stress is not being aided by smoking/alcohol/coffee and told her to stop all three in whatever order she finds most manageable. She has no problems quitting the alocohol, coffee was harder, smoking hardest.

The tinnitus was more of a ringing type but the volume has been decreasing with time.

I am hopeful for both, but I have never seen anything like this before. More hopeful for regulating digestion and decreasing stress.

sionneau lists the following patterns for hair loss - yin xu,

qi & blood

xu, wind due to blood heat, blood stasis

(The treatment of disease in TCM, Volume 1,Blue Poppy,page 87-88)

 

His formulas are not standard and the material is copyrighted, so I

can't reproduce any more details here, but you might get a copy and

check it out.

Sean,

I have good results treating female alopecia using a variation on

" alopecia pills " . To your formula I would add qiang huo 3, nu zhen zi 5,

han lian cao 5, dan shen 5, remove the tao ren and increase the he shou

wu by 3-5. I'm not sure the qi ju di huang tang is necessary at this

point but the xiao yao wan or elements of it should be good for the

liver related digestive and stress issues. I have always had at least

moderate results with this although I've never had a case that included

loss of body hair.
