Guest guest Posted June 4, 2000 Report Share Posted June 4, 2000 Lets try using a standard case submission format as Zev suggested before starting a separate list, as the majority opinion seems to be for not breaking it up right now and I also feel that way, at least tentatively. Feel free to edit this format, but this should be the minimum. I have also noted how these TCM paramters fit into a standard SOAP format. 1) Chief complaint (this should usually be a symptom, rather than a disease; it should be elaborated using OPQRST) (subjective) 2) Biomedical Dx (if available, include dx method, i.e. lab test showed elevated liver enzymes or intestinal parasites, etc.) (objective) 3) Relevant Medical History, include habits, diet, exercise, etc. 4) questioning examination (subjective) 5) pulse and palpation examination (objective) 6) tongue and observation examination (objective) 7) the pattern(s) and treatment principles you chose, including your rationale (unless this is your question) (assessment) 8) herbs used (unless this is your question) (plan) 9) other treatments or lifestyle changes (i.e. patient fasted for ten days or began to exercise regularly) Quote Link to comment Share on other sites More sharing options...
