Guest guest Posted June 21, 2000 Report Share Posted June 21, 2000 Greetings, I recently am seeing female patient, 33 yrs. old, who has been diagnosed as having endometriosis - " chocolate cysts " on her ovaries. She is also trying to get pregnant and has not been successful after 12 months. SHe is in relatively good physical health - eats well, sleeps well and has fluctuating energy. Overall her body temperature is even, with an increase in body temperature at night (although no night sweats, just a bit hotter than during the day). Family history of endometriosis and difficulty in getting pregnant. Has had abdominal pain in Nov 1999 (sharp, stabbing) until January 2000, then again in Feb. IN past month (since treatment), her pain has decreased to a dull, achey sensation, worse at ovulation. Menses were dark brown blood for first day, then red with small clots and increased cramping for days 1-2. Some PMS sxs of irritability, breast tenderness, bloating, headache. Ususal flow is 3-4 days. Tongue - pale purple, sl. puffy with thin sl greasy coating in back Pulse - R - slippery, sl wiry L - thinner than R, more wiry, deep Ki I have given her gui zhi fu ling wan, which she has been taking for the past three weeks. I want to further fine tune her treatment and am looking for additional suggestions. Guest guest Posted June 23, 2000 Report Share Posted June 23, 2000 Susan Looking at the signs and symptoms , there is clearly blood stasis and liv qi stasis.You may want to concider stronger blood movers ,hong hus,tao ren,san leng,er zhu,wu ling zhi and pu huangAND qi movers yan hu suo ,chai hu yu jing.For breast tendernes I was taught a couplet gua lou ren and bai zhi. In treating infertility have you heard of the technique if its a 28 day cycle .....that on days 1-4 you use herbs to regulate qi and blood days 4-12 tonify blood and yin and fill chong channel days 11-13 tonify yang days 13-16 regulate qi and blood days 16-25 warm and tonify yang days 25-28 cross your fingers I got this from Jane Littleton (co author of TCM clinical handbook)and specialises in TCM infertility. Hope it is usefull. Guest guest Posted June 28, 2000 Report Share Posted June 28, 2000 Hi Susan, In regards to your infertility patient, does she chart her periods with her basal body temperature and cervical fluid? First off, If she does not have good cervical mucous at ovulation think about her yin. If she has not charted, I would have her start right away. If you have not used temperatue charts to help diagnose in TCM, let me know and I can help you interprete her charts according to TCM. I highly recommend the book, Taking Charge of Your Fertility by Toni Weschler to any practitioner (or patient for that matter) who is treating infertility patients. It is the best explanation I have seen on acheiving or avoiding pregnancy using Natural Family Planning and will terach you how to interprete charts. I find charting invaluable for infertility diagnosis and monitoring a patient's progress with TCM. I have tried gui zi fuling wan for chocolate cysts in endometriosis a few times with out success. I think the blood moving herbs are a good idea, but in endo, watch her blood and yin, it is more often than not a mixed pattern of excess and deficiency. I have found that advising the traditional no damp diet does seem to work fairly well if the patient will follow it.
