I prescribed Xiao Yao San to a patient, Mayway patent, because she wouldn't take raw. Typical liver qi constraint with mild liver blood xu and spleen qi xu. (I also gave her curin wan for motion sickness, which she originally came for). She now wants to be my poster child for wonderful menstrual periods. However, after being on the formula for about 4 months, she swears that it led to a decrease in the buildup of tartar calculus when she went for her six month dental cleaning. Anyone have a differentiation for calculus? If teeth are the odds and ends of the bones (and therefore a kidney tissue), is calculus a kidney xu problem? Or, is calculus a buildup of dampness from spleen qi xu? Second, I have a 13 year old patient with ITP (ideopathic thrompocytopenia purpura). According to the Merck Manual, in children, it is thought to be the result of a viral condition, which will spontaneously remit after about 6 months. In adults, it is generally thought to be a problem of immune malfunction. The body stops producing platelets,leading to hemorrhage if unchecked. (Here's a good reason to be able to read labs, so I can tell if my treatments are having an effect). This young woman has had it for over 1 year, so they think it's not viral in nature. Problems arise in differentiation. The ONLY symptom she has is bleeding/bruising, every third or fourth week as her platelet count declines. Her tongue is pink, moist, and DOES have a moderate to thick coating with some red dots. So she does have some spleen qi xu with a damp accumulation and a little heat. I don't know if this is from drinking 2-3 glasses of milk a day with the occasional ice cream or the disease. Easy bruising is a sign of spleen qi xu, afterall. Pulses are a little soft with chi on both sides being kind of weak. I could treat her for spleen qi xu, but first I'm gonna have her parents clear her from dairy for about 2 weeks and see what happens with her tongue. I'm also considering 7 Forest Lithospermum 15, which Subhuti has formulated to treat immune function disorders (plus there's no way I'm going to be able to get her to take a raw formula). Has anyone else ever treated ITP in children or adults before? One last point. I know there are a lot of immune disorders out there, many of them manifesting in women primarily. I know from a western perspective they have different causes, (maybe), but I'm beginning to wonder, energetically and from a Chinese bian zheng differentiation, whether or not they have a similar cause, but manifest differently based on body constitution problems. For example, one patient gets rheumatoid arthritis, the other gets lupus, the third gets Hashimoto's Disease. Anyone have any thoughts on this? Thanks in advance for insights. Jeff P.S. I personally do not want the responsibility for ordering labs, but I sure as hell use the results when their available, particularly for issues like treating male and female infertility.
