A friend just did this with a West Coast company that does some distribution. When ordered, the book which is on computer, is printed and bound. The book is never in print so it doesn't go out of print, and inventory costs are close to nil. He orders a small batch before seminars where he may sell it in person, links the publisher's order form to his web site, sells it through Amazon, B & N and some bookstores and uses his own mailing list developed over years of giving seminars. Shipping is slightly delayed, but a good quality 120 page trade paperback retails for $18. The author is compensated as sales take place. Hiring a good editor would be extra.
