Shu Gan Wan

Does anybody know for how long we can use " Shu Gan Wan " ? For one month

two or more.My patient has pancreatitis and problem with his

gallblader and liver.When he has dull, distending pain on the right

hypochondrial are he has at the same time a strange feeling on his

tongue: some kind of burning sensation like after drinking a hot tea.

His tongue is purplish with thin greasy white (sometimes slightly

yellow) color. Tip is red/ The body of the tongue is flabby.Pulse -

choppy, slippery.He feels distention usually after meal, and I gave

him Shu Ganwan. He feels better with the formula, but I am not sure

for how long he can take it. Any idea and explanation?

Shu gan san I think addresses more specifically liver qi stasis /period pain

type problems.

Your patient with the red tongue tip would indicate some heart fire /heat

etc,the flabby indicating possible damp and purple :blood stasis which

shu gan san doesn't address ,in fact the drying nature of the formula may

damage the yin xu if there is any.

The symptom of the strange burning sensation is interesting > the closest

thing that I could think of is that it is due to liver and kidney yin

deficiency with concurrent liver qi stasis .Have a think about the formula

yi guan jian which can address stuff like distention with concurrent dry

mouth .

Questions:

1) what part of the tongue is effected?

2) What are the GB/Liver problems?

3) What are his digestive symptoms?

 

 

If the pulse is truly choppy and slippery, it would indicate possible liver

blood depression and dampness. In which case, another prescription should

be considered. You seem to have a pattern here of mixed vacuity and

repletion, so you probably need to stop the formula soon and use a

prescription that is more appropriate.
