One thing that comes to my mind is that when a disease involves latent evil (fu xie), the acute sx tend to surface when the zheng qi is strengthening and attempting to expel the the fu xie through the surface. When the zheng qi is very weak, the fu xie remains trapped and no acute symptoms appear.
Guest guest
Also consider another way to look at it. Symptoms are created by significant contrasts between different parts of the body, systems, muscular groups, etc. People will still have a lot of things going on without symptoms being present. In these conditions, the immune system is trying to overcompensate and is able to repel more acute problems or there isn't enough zheng qi to help create that contrast. Which is why the acute development of symptoms (like cold, etc.) can be a good sign. We can often see that contrast in the pulses before symptoms set in. And the same is true for people at the opposite end of the spectrum. Things can be developing or exist without symptoms presenting. For example, the growth of tumors, long buried psychological stresses, etc. Latent evil (fu xie) in terms of bacteria, viruses, and environmental toxins are always present. How well the immune system functions (in 5- elements it's the spleen, lung, and kidney marrow functions in combination) determines how well things are kept at bay. Yin Chiao is useful for superficial, wind-heat types of cold and flu. But over the last 10 years, I've noticed that colds and flus move from the Tai Yang to deeper levels inward more quickly (and don't always follow the sequence in the Shan Han Lun). Macioca's recommendation would be useful if the patient notices the Tai Yang symptomology right away and takes Yin Chiao then; otherwise it is not that effective. Every several years I reformulate our Cold and Flu formula to try and keep it current with the changing energies of the year (see Su Wen chapters 66-74). Last season, we went through a modified version of Xing Su San more than anything else (2000 was a metal replete/Dragon year). When patients report a cold or cold symptoms, that means that there is enough wei qi to mount an attack giving rise to the symptoms of the superficial pathogen. In the absence of wei qi, the pathogen just goes in deeper leading to other client reported complaints.
