I checked the Herbacademy archives and did not find anything, as I do not have a subscription to Itcmonline, nor the book referenced! Female patient, 55, presents with Hot/Cold & sweaty (had this symptom pre-detox from western pharm. phentonal patch, clonopine, actophine). Western Dx. is Neurological Lymye disease. She is having her 4th relapse in 12 years, and this one is the worse. Chief Complaint: Nausea, severe cramps in both legs, bilateral, 24/7 (calf muscles feel wasted or non-exishistent)... other complaint is Hot/Cold * - 0-energy * No sleep at all for days * -0- appetite -- 63 pound loss since June * Everything* makes her nasueated, including water * no BM and little urine because does not want to eat... because it triggers the nausea. Sometimes burning urination *In Menopause ( on HRT) * Emotional -- feels depleted and present this way, as well * On the outside body feels warm to touch, however on the inside VERY cold. Tongue is pale, with a couple of yin type cracks, very thin to no coat, wet Pulse: slow, Xu, first 2 positions felt slippery and wiry... is that possible? Also right proximal was the most deficient, with the left proximal feeling the deepest. I have the opportunity to work with her 3 times a week for the next month... so would love to see her feeling better! She has had acupuncture in the past and raw herbs. Presently she cannot tolerate any raw herbs. I am still contemplating my dx. Definitely Spleen Xu with spleen not nourishing the muscles; Kidney yang and yin xu, blood xu I want to get to the root... I agree about the sleep, no BM and urine pain. I think she was giving me the complaints, that she felt was worse. I found out the other stuff by questioning. I could see the questioning was tiring her... and it was apparent that she was dehydrated, which makes me think her electrolytes are out of balance and may be the cause if painful urination *lack of fluids* Another thing she gets, however does not have presently is bi syndrome... she said, very severe joint pain. Alon, she is withdrawing from those pain meds! Apparently she is not in as much pain is before, presently calf and leg cramping (this was pre withdrawal from the meds), and I believe pain from withdrawing. It also looks like the original contraction of an exterior evil, combined with treatment, damaged the yang qi of the spleen and kidney. The yang is floating to the exterior, as in a tai yin disease, however, the slippery wiry pulse shows possible damp heat depression as well. I have used The Tx of Modern Western Diseases in book, just put out by Blue Poppy as an adjunctive text. I will give my thoughts on your case tomorrow, Saturday, when I have a moment! So far I have used only the Japanese Sawada Tx. and could only do her Yin side before she got to restless. The Physical Therapist did some massage work on her back... as that day she was having cramping, in the back and both legs. I am afraid to do to much needling, if any, until her constitution is a bit stronger. Here is a bit more about her presentation. The second day I worked on her she had some chills... the Hot/Cold seemed to be gone or markedly decreased (makes me believe it was part of the detoxing?) Forcing herself to eat a bit..knowing it will be followed by nausea... also gets very nauseated with any strong smells. The tick bite went un-diagnosed for 2 years, and thus this is why all her symptoms are very extreme. Tongue: where the first day,her tongue was very pale... second day it was red and dry... with thin white coat, yin type cracks are more to the right side of tongue, nearest the lung area. Pulse was very down, in the R proximal, Slippery in the R Middle & Distal position. Left proximal could not feel anything, L middle/ liver was wiry and the L distal/ heart was shi. Other signs... twitch in right eye, has had Bell's Palsy relative to the bite When relapsed occur she goes on a 8-10 week IV Rosiferan... Antibiotic. The flare up up began in April. I will get a list of Western drugs on now. Do not have them with me. I am scheduled to see her on Monday & Tuesday. Thanks!
