46 yo female

chief complaint 1: colon cancer prevention

cc2: hair thining

 

This patient initially came into my office complaining of receding hair one

month ago. She has changed her priority in light of her mothers 2nd

diagnosis of colon cancer to colon cancer prevention due to its hereditary

nature and she has suffered from internal hemoroids and continues to as

evidenced by occasional blood in the stool coinciding with heavy lifting or

constipation.

 

She is also on a regiment from her MD to treat heavy metals. She haas high

Lead in her blood and is taking the following until July when she is tested

again.

 

Multiple, C-Estra, B-5, Oral Chelation, Lipoic Acid, Thyodex, Cholerest for

2 months.

Other meds: Zoloft: 1/4 dose due to her " high sensitivity " to medications of

all kinds.

 

Career: Conserves paintings and architecture and is exposed to solvents etc.

Does not always wear gloves and or mask.

 

Sleep: wnl

Emotions: " black moods " once per month. Zoloft has helped

Energy: 2 years ago = exhaustion,. Went to endocrinologist = WNL

Appetite: sometimes low but overall OK

GI: some tuberous gas

Diet:Balanced***open for suggestions for colon

BM: 1 per day = formed

Urination = WNL

Menses: some pain before, little heavy, red blood, no clots,

Circulation = WNL

Other notes : Cholic as child

 

Pulse:

Qi: tight

Blood: heat

Organ: tense, slippery

Rate : 70bpms

Diaphragm: Left =yes, Right=NO

Esophagus: tense, tight = patient reports tight St-9 area

Spleen: Inflated

Lower jiao: Left: tight Right: tight

Lower body: Right :Absent?? Muffled?…fibroids removed

Left: deep

 

Tongue: Small body, Pale, slightly puffy, central crack to lung area where

it stops and another crack starts and runs to the tip.

Heart area: red spots..not too excesive

Lower jiao : dry sticky coat..almost like antibiotics with Fur that runs

into the central crack

 

Formula 1: Complained too sour made stomach upset almost immediately. I

focesd on the hair loss primarily.

Liu Wei di huang wan +

Gou ji zi

He shou wu

Tu si zi

Ce bai ye

Han lian cao

Nu zhen zi

Sang shen

Gan cao

 

This formula was in GRAULAR FORMULATION.

I want to shift to focus more on the colon cancer prevention

CASE 2:

Patient 2: 65 yo female

Cc: rt shoulder/neck pain

Cc2:borderline high bp

Morning: 159/95

Evening: 127/79

Cc3: short term memory fading

 

All signs WNL except:

Excessive urination..numerous times per night

 

Pulse:

 

Qi: tense, tight

Blood: heat?? Possibly thick

Organ:tense, ropey, slow

Upper: tense slippery

Mid:slippery, yield, slow

Lower: tight

Left: slow, rolling?

Right: tight, vibration

Pc = present

Middle jiao: left: borders= hard, easily discernable

Middle: right: tight, inflated

Spleen: tense, inflated pounding

Lower jiao: left: deep tight

Right: tight tense

Lower body: right, left, muffled, slippery = full hysterectomy, fibroids

removed

Tongue: deeply carved scallops, deep central crack

 

Ideas: massive sp qi def, blood heat or thick and the walls of the artery

are hardening (see middle jiao) and narrowing reducing blood supply to the

brain affecting memory.

 

Is this " idea " part realistic or am I off the deep end crossing paradigms??

 

Herbs: Tian Ma gou teng yin?

Acupuncture: weekly for 3 months? What do you all think?

strain, bearing down causing blood loss makes me think qi

sinking and not holding, but I suspect dampheat a factor, too

pulse, tongue coat suggest dampheat or phlegm heat.

dampheat is a very common complication in many patients. I

expect to see a symptom that could involve dampheat (like

bleeding hemorrhoids). Something is up with the moods. pulse

suggests liver qi, but it could be liver blood xu as the cause. The

fact that they not constant further suggests liver involvement.

With the fatigue, gas, hemorrhoids, it seems there is some

spleen involvement. At 46, if she has not been attending to her

diet all along, spleen xu is almost inevitable.

 

The formula leans towards cooling and nourishing. I don't think

it sufficiently regulates the liver/spleen or addresses a potential

pathogen (dampheat?). Failure to do this will result in her being

unable to digest the herbs and they will upset her stomach.

 

To focus on colon cancer prevention, I think you need to identify

the essential patterns that need to be addressed. In the early

stages, emphasis should be on regulating digestion and

removing pathogen. Then you can get a clearer picture of what

type and how much tonification needs to be done. However, at

the first stage you must consider how to protect the yin and qi.

Because the vacuity is clear. And at the later stages, you must

consider how to keep the system free of pathogens. It is more of

a shifting emphasis over time than a complete change of

strategies.

Hi Kelly,

I'll give you an 'experience' gastritis formula we used in clinic for a

gastritis patient, of course, modify or throw out at your discretion! I

thought it might be interesting since this gastritis patient had abd.

pain after eating and family hx of cancer in the digestive system - and

the supervisor made a point of asking about that with gastritis

patients. She also told the patient that she should use 2 bags of the

herbs forever to keep the gastrits in control and to prevent cancer.

 

tai zi shen 30gm

yi yi ren 15

e zhu 15

huang qin 10

xian he cao 15 - this herb is good for Spleen Damp! I never heard

that before either..

omit these two if has ulcers (sour flavor not good for ulcers):

*shan zha 20

*mu gua 15

bai shao 24

gan cao 9

add for cancer prevention:

* pu gong ying 15

* bai hua she she cao 15
