2 challenging case studies

thoughts and insights are greatly appreciated.

 

I am perplexed by these cases.

CASE 1:

 

35yo female

cc: Insomnia diff falling asleep

cc2: Exhaustion, but unable to sleep

 

ss:

mild premenstrual signs

meds: sleeping pills for two years straight

currently:serazone and ambien

cold hands and feet.

coffee 2xs day

histroy of cocaine use

Wood /Fire CT

Herbs: yunnan pai yao 1 week, gui pi tang 3 weeks with An shen bu xin wan

(patent)

Findings: Major erythema on back shu points, Have done it three times and am

mostly treating back

points now : Heart, PC, Gao huang shu, with accompanying distal points.

 

Pulse:

Overall:thin, tense, tight vibration deep

qi: yielding

blood:spreading

organ: tense

upper:tight

mid:spread:

lower:slow? reduced substance

rate:58

end:60

movement: 66

pc:present

diaphragm:present

leftmiddle: no organ depth??

rightmidd:spreading

lower left: deepweak

lower right: tense slippery

lower body:

L:tense slippery

R: cotton

 

TOngue:

thin pale with purple patches on liver GB area/sides, deep central crack,

sublingual distension +2,

family: breast, lung cancer = maternal grandmother

ps-the language of the pulse may only be familiar to some but anyone's

observations and experience is greatly appreciated.

Case 2:

60 yo female.

cc1: burning pain in rt central area of back upon exertion

cc2:tendency towards very loose/diarrhea stool since young adulthood..=

30+years

cc3:approx 1 hour after eating sweets or carbs..almost is comatosed for 20

min then completely wide awake. this also may occur after exercise but not

always = 3 years

 

cc1: occurs with mild exertion such as cooking. rest makes it better

cc2: has tried food combination theories etc...it can happen almost immed

after eating or 30 min to 1 hour. there is no consistent pattern of foods .

cc3:no pattern

 

other significant findings:

vegetarian for 6 years and as a child rarely ate meat = current

conjunctiva = very pale +2

alcoholic 20 years sober: for 2 years drank heavily, constantly but was

functional.

approx many years ago before first child (40 years)smoked 4 packs of

unfiltered cigarettes per day

for 5 years

claims " i must do everything in excess "

1990: mastectomy right

menses histroy: heavy bleeding with clots

 

Pulse: overall tight-tense with reduced substance at the blood level

pc = present

spleen:inflated

massive diaphragm pulse

 

rate:59

end:62

with movement: 72

 

tongue: pale. mild heart crack = Overall PALE

Seems to be a few elements of blood stasis from what you have

presented. Is this the entire case? No other data on digestion,

urination, menses, etc., etc.? Blood stasis is often implicated in

very stubborn depression. Xue fu zhu yu tang is indicated for

this pattern. I often find confirming signs and symptoms and

have good results if this is the case. Blue poppy has recently

introduced a formula that addresses this pattern as well as its

typical concomitant paterns, such as liver depression and heart

blood vacuity. Your patient has some s/s of both of these as

well. I am always careful not to overlook the vacuity and often

combine xi yang shen or white ginseng into the formulas. both

seem to improve energy without disturbing sleep.

while this is true, it is hardly relevant. blood stasis is also

indicated in very stubborn insomnia and that is what the BP

formula is for.
