He claims the opening was reduced to 1/3 its normal diameter. Has anyone ever heard of this. So I ask him if he is experiencing any pain. He says, " a little. " I ask if his urine is inhibited. He says, " not anymore. " I say, " not anymore? " He says, " Yeah, I just cut it back open with my exacto knife. " Do I have a patient with a physical condition or a mental condition or both? And assuming this is on the level, what are the possible pathomechanisms for this condition (the penis thing, I mean; I am quite clear about the pathomechanisms of delusional behavior, which I have not yet ruled out).
