I have been seeing quite a few impotent men lately, so the topic is timely. One of the things that is common in middle aged men is as my patient last night put it, " the spirit is willing, the flesh is weak " . In other words, all these men have interest and desire, but not function. They all appear depressed, usually overweight with evidence of some form of stagnation (blood, qi,phlegm). In some cases they are obviously yang xu, but that has not been the rule. Marnae will be lecturing on " beyond kidney yang " . However, upon investigating formulas for impotence, it seems that yang tonics are almost always used to some degree regardless of pattern (see sionneau for example). I have seen this same approach with infertility, where yang tonics are always used at certain stages regardless of whether overt yang xu is present. Now in simple yang xu impotence, the treatment is straightforward, but I often find more complex cases. I wonder what people's results have been with impotent patients where stagnation predominates and do you typically use yang tonics in such cases regardless. This may be a clear example where bian bing as is important as bian zheng. It is unclear to me whether merely dispersing the stagnation would be effective. I have not seen any dramatic short term results just treating the stagnation in the past. Conversely, I have seen patients injured by indiscriminate use of yang tonics. I look forward to Marnae's presentation as she has thoroughly researched this matter in chinese sources.
