Sheer guesswork ... but how about this...

 

Could it be that he defecates with lips closed and greatly increased

pressure in his mouth? Could this increase blood pressure in his

mouth/tongue/larynx/pharynx, thereby engorging the tongue? Could

it also allow regurgitation of bile to the mouth?

 

If yes, ask him to keep his mouth wide open when he defecates

and see if this makes a difference.
