My patient is a 53 yr old female who was diagnosed with proctitis 3 years ago and was successfully treated with Mesalamine (anti-inflam) at that time. Last November she was diagnosed with stage 2 colitis. My TCm dx so far is: Liver attacking Spleen with Damp-Heat in LI. She is hot to touch and has a feeble pulse that is barely palpable after having just come off 3 months of prednisolone (at first (30 and then 40mg on a reducing dose since March 2002.) Prednisolone (corticosteroid) has had little effect on her LI symptoms eg. explosive bloody diarrhea, cramping pain in LLQ smelly flatulence, creaking noise from bowel and bouts of extreme ravenous hunger and tightness in the LI area. Her tongue has no cracks, a slightly purple hue with a yellow coating in middle and back. She get sores in the Liver/GB area of her tongue occasionally. Her symptoms are triggered by stress and definitely worse pre-menstrually. Her specialist has told her she needs to go on Azathiaprine (imminosuppressant) for upto 10 years with regular blood tests or she will have to have her colon removed. He has frightened her into thinking this drug is the only way to go, but she has come to me as a last hope. I have put her on a dairy free regimen and told her to avoid spciy, alcohol, raw and the usual dampening and heating things. She will start a food diary, explore new ways to control her stress levels and will take raw herb decoctions from next week. By the way, she is vegetarian. Can anyone suggest anything else?
