The fellow with the avascular necrosis has about a dime sized piece of the top of the femur that has died. I have no other details. Avascular necrosis refers to a progressive disruption of circulation that eventually leads to a collapse of the bone structure, if not treated in time. It is seen in children and young adults as the result of injuries and various genetic bone disorders. Later in life it occurs as the result of drugs (most often from corticosteroids and alcohol) and secondary to chronic diseases (e.g., systemic lupus) that affect the vascular system, or a history of injury. The most common joint to be affected is the hip and is seen in a patient complaining of pain and restricted motion of the hip. The pain is worsened with activity and improved by rest; one or both hips can be involved; in cases other than traumatic injury, one side is affected first, and the other may develop the condition later. Treatment: In the early stage complete rest and core decompression may work and prevent the need for joint replacement.
