I bumped into this case, about 3 wks ago, and would like share it with the group. this is a 34 year old male who is going for his 3rd kidney transplant. He told me a long story which I'm gonna try to cut it short here. Ok, this guy undertook his 1st transplant when he was 20 yrs. old. 32 days after the surgery, the new kidney stoped working (there was no explanation, bad meds???? That's what he thinks). the 2nd transplant happened 2 yrs later and now, after 9 years, the new old kidney is failing again, working only at 40% of it's capacity. Then he decided to try . Usually I follow the Chinese way, I mean, I do the diagnosis, pattern diferentiation, treatment strategy and so on.....But since this is an interesting case, I'd like to hear from my coleagues. He complains of tiredness, numbness and tingling on his hands, legs weakness, nausea, anemia, impotence, insomnia, headache (occiptal) and high BP. He is more sensitive to cold weather, sweats a lot whenever his BP goes to normal levels, avoids drinking water (fear of edema), has no salt in his diet (eats lots of garlic and lemon). Urine is, according to him, less frequent than normal but with increased volume. Color is slightly yellow and he also feels some dull pain in the lower abs at night. Intestines work twice a day. He also said that once in a while he gets some purplish spots all over the body. Has cold extremities, he's losing muscle mass and almost every wk he gets some facial edema. Because of the high BP (155/10) his heart is a little bit enlarged. He also lookes a little jaundiced to me His tongue is sl. pale and puffy, with tothmarks and thin white coat. his L Pulse is very deep (hidden) and the R. Pulse is deep and thin. OK, I only saw him in class, a student brought him in for a diagnosis class. His in line for his 3rd transplant. I have something in mind regarding the Tx but I'd like to hear and discuss with you. thank you JPaes SBC - Internet access at a great low price. He told me he was doing something on the roof and fell on his back. one year after that he was doing his daily jog and felt some sharp pain in his lower back. After waiting for a while he went to the doctor who told him he had a Kidney infection. After some antibiotics the pain disappeared. A couple of months later everything came back and both of his kidneys were dead.
