Guest guest Posted September 1, 2004 Report Share Posted September 1, 2004 Have been treating a 17 year old with dysmenorrhea - she started suffering the pain 12 months ago - then started with an irregular cycle bleeding every 14 days for 14 days. After 3 months the pain got so bad that she started Ortho Evra as a patch. This regulated the cycle but the pain remains in the area over the left ovary. The pain occurs 1-2x per day through the whole 28 days of her cycle. The OB suspected ovarian cysts, but the ultrasound was -ve. No other testing was done. There are clots present in her cycle. After 3 months on the birthcontrol patch the pain continues, both over the ovary and in the low back 1 day before and 3 days of cycle. It is sharp and tends to run down both the side and back of her legs into the calf. Patient has a low affect, low energy level. Has difficulty falling asleep till after 2 am then unable to waken easily. She experiences vertex HA's almost daily. BM's alternate days but consistent. She is strong willed and has a quick temper. Her pulse is rapid and wiry. The tongue is pale pink, with a white coat and slightly shaky - there is no sign of stagnation or engorgment. On palpation there is tension through the Ren Mai CV 14 - 8, reflex tenderness over left Zigong, Along the channels there is puffiness and tenderness particularly at Sp 8 - tenderness at liver 8. 2 weeks ago the patient started Rx stratera for ADHD - to help focus. 2 Days ago the pain was so bad the patient woke at 2:30 with cold sweats and pain - a visit to the OB, they have taken her off the patch and want to try Depo-Provera to arrest the cycle for 12 months. Failing that they want to do investigative laproscopic Sx to see if endometriosis is the cause. Question Do I continue to treat while the patient is taking Depo-Provera. Would an herbal regimen be suitable to try and treat the root (damp/phlegm acc, SpQi Xu, Lv Qi constraint) The parent is looking for advice - understanding that the Depo-P is only masking the problem - but also seeing the pain and suffering that the patient is experiencing in her Sr year. I have requested medical records and in particular was looking to check the Thyroid. My only other thoughts were appendix or possibly Kidney stones, but there has been no burning or change in urinary symptoms. And the Ultrasound back in January would have most likely shown stones or enlarged appendix. Since she has pain over her left ovary treat it. One of my favorite techniques for this sort of pain is needle above zigong and needle Sp 8(which just happens to be tender how convenient!) connect them with micro-current. Works great.
