rosacea blood heat and bladder infection

Hi All

 

Has anyone ever seen something like this?

 

Patient, female 40, has rosacea with thickened facial skin, slight

redness on nose and cheeks, one area of erythma on right cheek.

Feels hungry and eats (vegetarian) all the time but is skinny. Has

cold body, light menses, headaches at yin tang, anxiety,

perfectionist, depression, premenstrual acne on chin, frequent

urination, lower back pain. She has a history of bladder infections

previous to the rosacea which became chronic and treated with

antibiotics which she finally abandoned in favor of uva ursi

(cranberry did't work). With these she gets the urge to urinate

without the ability to urinate. Her pulse is thin and wiry with

weaker deeper chi regions, 56 bpm and tongue was dusky and pale with

a slightly red tip and thin white coat. I had never treated rosasea

before and found a protocol suggesting LI4, LI11, yin tang, GV 25,

LI20, St4, CV 24, SI 18. Her diagnosis was St/Lu heat in blood

level. I am confused by the seeming predominance of yang xu symptoms

and signs. Problem: a week later she got a bladder infection with

blood in the urine. Now her tongue is red, pulse is wiry and 80. My

question, I guess, is how can someone who is seemingly so cold with

pulse of 56 have heat in the blood and how to see this and treat

this concurrently. Can I give her Xiao Ji Yin Zi for the Lin

syndrome or will it damage yang? Should I use Zhi Bai Di Huang Tang

instead? There's thin to no coat on the tongue and she rushed

straight to the MD and is on antibiotics for the infection. He told

her she probably has had it for quite a while and that it has just

been hanging around and subclinical, coming and going. How does this

relate to the rosacea? Could the rosacea have come about due to

supression of the original bladder infections with antibiotics? Have

I stirred something up and is this a bad thing? Any help would be

greatly appreciated.
