I used differentiation under the disease category of vulval pruritus. Without going too deeply into the mechanism essentially it is due to kidney and liver yin vacuity with wind and dryness present. Because of the dry and damaged skin continually sluffing off the body builds new tissue in response and then that new tissue can accumulate to such a degree to obstruct or in some cases nearly fuse the vaginal opening. In terms of pattern differentiation I look for Ki and Lv vacuity, blood vacuity, wind. The formula I have had most success with is good ol' Zhi Bai Di Huang Wan.
