I have a 49 year old patient that is suffering from two uterine fibroids. One of them on the left is 4 inches in length size by two and the other on the right is two by two. This women had these before and they were surgically removed unfortunatly they grew back. I have been treating her now for two months and feel that the fibroids have not changed much in size but have moved lower about two inches in location. This patient is a very busy lady overworked with her job and she suffers from fatigue. I am wondering if these are just too advanced to reduce and get rid of them? They can be attributable to any number of patterns and, of course, combination of patterns from a Chinese perspective including liver Qi stagnation, blood stasis, toxic heat, phlegm accumulation, or Qi and/or blood vacuity. Chinese medicine and acupuncture work slowly, but they can be very effective in resolving fibroids. However, from research I have done, and my experience, emotional issues must be dealt with as well. I have personally seen and read of many cases of women wanting so much to have babies, that their bodies have imprinted this message, and unless they can release it, they will continue to produce them fibroids. Guest guest Posted August 8, 2007 Report Share Posted August 8, 2007 Without any TCM dx info to go by, difficult to give any specific recs. However, in general, I have had good success in treating uterine fibroids. The fact that a woman has uterine fibroids is not in and of itself problematic. The issue becomes what symptoms/problems are they causing? For example, if the patient is having excessively heavy bleeding for long periods of time with little break between menses then you must address the bleeding first and foremost, imo. Also, make sure she is not becoming anemic. (My assumption here is that you are in a sense " co treating " the patient as she is most likely under care of her gynie as well.) Use your TCM diff dx here. In addition to the usual suspects, look for any evidence of shi & /or xue heat. I always incorporate customized herbal therapy into their treatment plan. Also, give it at least 3 complete cycles to determine efficacy (although you should be seeing evidence that you're headed in the right direction as you move along). I usually treat w/ acupx & herbs for the first 4-5 weeks and then continue with herbal therapy for the next couple months with acupx scheduled as necessary depending on the patient. Also, sometimes uterine fibroids can cause a structural problem - for example their size/location can interfere with bladder evacuation contributing to incomplete voids making the patient more susceptible to recurrent UTI's. You'll need to address that symptomatically but can also use that as a benchmark to see what/how the fibroid may be changing as a result of treatment. Remember to refer if necessary. (as an aside, I keep urinalysis test-strips on-hand for quick in-office diagnostics.) On the extreme end of things, fibroids can grow so large as to out-grow their blood supply, becoming necrotic. This is a surgical issue. The necrotic tumor can become gangrenous and is of course a critical situation. Of the cases I've seen, the patient usually exhibits prodromal symptoms such as unexplained fatigue, an vague sense of feeling " ill " but not really able to define why, and possiblely abdominal/pelvic discomfort. I don't know if that presents in all cases. Refer back to the patient's gynie for a r/o dx if at all unsure. Keep in mind that in many cases, if a woman is " close " to menopausal age and the fibroids aren't causing any critical problems, many MD's will just suggest a " wait and see " approach hoping that as estrogen levels naturally decline, the fibroids will begin to shrink on their own. We really have so much to offer these women!! Don't be afraid to treat - just be safe and make sure all appropriate follow-up diagnostics are being done if necessary.
