Guest guest Posted February 19, 2010 Report Share Posted February 19, 2010 I'm wondering if anyone has seen Chinese herbs resolve or at least improve a large ovarian cyst. I seem to recall one of my instructors telling us that if a cyst is over 8 cm, it is too late for herbs. Does anyone have any experience they can share? The patient I am working with has a complex cyst of the right adnexa that measures 8.1 cm x 11.9 cm x 8.5 cm (by US on 11/10/09). Thank you, Julie C. Espy, L.Ac. I did have a woman come in with a " fibroid " about 3 by 5cm that went away within a week with acupuncture and a formula. We figured that it must have been a cyst that burst painlessly or we got lucky and it dissolved. It was a pretty standard lower jiao blood moving formula if I remember. I think I posted that case on CHA and Ray replied. It was about 6 months ago. If your patient’s fibroid was actually a cyst, then I am glad to know herbs and acupuncture are effective up to 3 by 5 cm. Has anyone any experience with over 8 cm cysts? Guest guest Posted February 22, 2010 Report Share Posted February 22, 2010 Hi Julie - As I'm sure you know, there are basically 3 types of Complex Ovarian Cysts: 1. Dermoid Cysts (these develop from the cells that produce human oocytes, and often have human tissue such as hair, teeth or skin); 2. Endometriomas (aka " Chocolate Cysts " - these develop from endometrial tissue implanting on the ovaries); 3. Cystadenomas (these develop from ovarian tissue, and are fluid/mucous filled cysts that can become quite large). Of these three types of Complex Cysts, the Cystadenoma is the most amenable to TCM. Endometriomas can be managed with TCM, but often do not completely resolve without Laparoscopic intervention. Also, because endometriosis implants on the ovarian cortex (the outer layer of the ovary where the oocytes are) - endometriomas should really be managed as completely as possible so as to prevent destruction of the oocytes, and prevent diminished ovarian reserve/infertility. Dermoid cysts have to be dealt with surgically. So, the size of the cyst is not as much of an issue, so much as the nature of the complex cyst. However, even Cystadenomas can develop ovarian " torsion " where they twist and cut-off blood supply, so if it is this type of complex cyst, and you decide to try to treat it, keep a close eye on the patient's symptoms, and if her pain level increases significantly, make sure that she consults her OBGYN asap.
