This verse is found in Sripada Ramanujacharya's "Sharanagati-gadyam". It is also found in the Mahabharata spoken by Gandhari to Krishna.

 

The Srimad Bhagavatam has a similar verse:

 

bhavaya nas tvam bhava vishva-bhavana

tvam eva matatha suhrit-patih pita

tvam sad-gurur nah paramam ca daivatam

yasyanuvrittya kritino babhuvima

 

"O creator of the universe, You are our mother, well-wisher, Lord, father, spiritual master and worshipable Deity. By following in Your footsteps we have become successful in every respect. We pray, therefore, that You continue to bless us with Your mercy."
