At the Mandir, milk is offered separately without any other ingredients mixed in. When offered to Shiva, here is the mantra that is chanted during the abishekam "Om Kaamadhenu samudbhuutam sarveshaam jeevanam param paavanam yagna hetushcha snaanaartham pratigrhyataam. Om Namah Shivaya paya snaanam samarpayami". Here is what it means "Om Coming from the ocean of being, the Fulfiller of all Desires, the Grantor of Supreme Bliss to all souls. For the motive of purifying or sanctifying this holy union, we request you to accept this bath. With this offering of milk for your bath Om Namah Shivaya" Hope this helps.
